Beginning the school year 2011-12, children in all grades K-12 in Philadelphia will need 2 doses of varicella (chickenpox) vaccine or documented history of disease. This is required by state health law in order for your child to attend school. Any child who lacks evidence of 2 doses of varicella vaccine or immunity should be vaccinated. In order to document that your child is protected from varicella, you should provide the school with 1 of the following items for your child's health record:
»Written documentation of 2 doses of varicella vaccine both given after the student's 1st birthday. Exact dates must be provided. Please have this form completed by your child's physician.
» Documentation of varicella immunity proven by laboratory testing
» Written statement of prior varicella disease from:
• doctor or nurse only for Grades K-1
